Change Status Request Form is required if there is a change of Address, SSN, or Name, Legal Documentation, i.e. marriage license, divorce decree, etc.
Confidentiality Form-Non-Release of Directory Information
Drop/Add/Withdrawal Form
Enrollment/Degree Verification Request Form
Intent to Graduate Packet
Letter of Good Standing
Request to Share Information forms must be accompanied by a valid photo ID if mailing or faxing the form to the Records Office. Students submitting the form in person must be prepared to show a valid photo ID.
